This Safe Work Method Statement (SWMS) outlines the main hazards and risks including the operation of plant, hazardous manual tasks, and risk of falls from heights in workplaces where Working at Heights, including handling of Ladders, Scaffolding, working on platforms, edge protection, Elevating work platforms/boom lifts, scissor lifts, fall restraint system and safety harnesses and fall arrest devices is required.
The SWMS provides details of the health and safety precautions (including personal protective equipment requirements) to be observed when working at heights.

Instructions for SWMS

A safe work method statement (SWMS) must be prepared for any and all high risk construction work to be undertaken prior to the work commencing. All high risk construction work must be carried out in accordance with this SWMS.
This SWMS must be kept and be available for inspection until the high risk construction work to which this SWMS relates is completed. If the SWMS is revised, all versions should be kept.
If a notifiable incident occurs in relation to the high risk construction work in this SWMS, the SWMS must be kept for at least 2 years from the date of the notifiable incident.

High risk construction work activities may include the following (A SWMS is required for all high risk work activities)

A risk of a person falling more than 2 metres (or 3 m in SA, or housing const. in Qld)
Work in an area at a workplace in which there is any movement of powered mobile plant
The disturbance of or likely disturbance of asbestos
Work on or near energised electrical installations or services
Work carried out in or near a confined space
Demolition of a load-bearing structure
Temporary load-bearing support structures
Tilt-up or precast concrete
Work on, in or adjacent to a road, railway, shipping lane or other traffic corridor used by traffic other than pedestrians
Work in an area that may have a contaminated or flammable atmosphere
Work on or near pressurised gas distribution mains or piping
Work on or near chemical, fuel or refrigerant lines

A generic SWMS may be prepared and used for high risk construction work activities that are carried out on a regular basis; however, the generic SWMS must be reviewed by the person carrying out the work to take into account the hazards and risks for the specific workplace and amend the SWMS as necessary for the site where the work is to be carried out, and complete details such as names of workers who will carry out the work. All amendments to the SWMS must conform to regulatory requirements and be recorded on the SWMS. Workers and their health and safety representatives (if any) should be consulted before the generic SWMS is first made available to them and all workers instructed in the SWMS by site-specific inductions or toolbox talks. Details of consultation with workers and instruction in the SWMS must be recorded on the SWMS for that project or site. All workers are required to sign-off on the SWMS before the work is commenced.

Potential Hazard/Risks	Falls from height, Crushing, Slip/trip.fall, lack of adequate lighting, weather conditions, Pedestrians, Sprains/Strains, Equipment failure, Unsuitable ground surface (roll over), Operator not competent/trained in use of (EWP), Contact with live electrical power lines/underground services. Obstructions (surface hazards/surround structures/vegetation.

Establish a barrier around your work area to make people aware that there is potential injury from overhead and to take caution. Ensure you establish a clear communication strategy with your spotter. Do not exceed rated load Wear PPE that is appropriate to the taks ( Eg: safety glass, enclosed sturdy shoes and head protection) Do not commence operations without completing the pre start check. Pay particular attention to all safety devises including emergency stop buttons and emergency lowering devices if so equipped. Do not use if EWP is not working properly or if any part is damaged, worn or missing. Inspect the worksite for obstacles around the work platform and in the path of travel such as holes, drop-offs, debris, ditches or soft fill. Check for overhead clearances. Allow for platform sway, rock or sag when establishing your work areas. Electrocution Hazard: The EWP may not be insulated. Maintain safe clearances from electrical power lines and contract with any electrically charged conductor. Do not drive onto uneven soft surfaces or slopes when platform is elevated. Do not sure without guardrails, mid rails, chain or bar in place. Do not stand on, sit or work outside of guardrails. Do not raise platform in windy, gusty or stormy conditions.

1. Tethering – tie off/securing tools, equipment and materials. Requirement: Tethers must be used to tie off Tools, equipment, and materials for work undertaken on all working platforms and for work undertaken outside of the edge protection on permanent or temporary structures. 2. Gaps on “Auxiliary” Elements – gaps in scaffold Requirements for fixed scaffold: No gaps ( vertically and horizontally) where objects can fall (except on working face) i.e must include fills, toe-boards, mesh on sides of all fixed scaffold. A structural engineers certificate must be supplied for all scaffold installed to these requirements. *Requirements for Mobile Scaffolds <4m: Mesh is not required to be installed, however, the following controls are required; a. only competent persons can install mobile scaffold b. mobile scaffold installation completion checklist c. toe boards d. tethering of tools and equipment. 3. Housekeeping* Requirements: A 3m radius surrounding the work area, which must be clear of unnecessary tools, debris, material or waste, unless they are tethered. A visual inspection to ensure compliance to this requirement should be undertaken to ensure compliance to this requirement should be undertaken at pre-start and end of works and before the scaffold or EWP is moved or adjusted. 4. Exclusion Zones Requirements: Exclusion zones must be in a place and maintained. These must be of adequate size and take into account the potential arc of a fall of material, deflections and bounce distances.

Job Activity	Working within a roof space
Potential Hazard /Risks	Electrocution, falling and trips
Risk Level	LOW
Working in a roof space should be kept to a minimum and only if a suitable alternative is not appropriate. Safe movement – Access strategy to be developed when working a ceiling void. This will include access into the ceiling and access while working in the ceiling. In an open cut roof space – MDF boarded walkways are a good safety measure to promote safe movement throughout the ceiling void depending on the volume and nature of works required. The decision to use boarding as an access strategy should always be assessed on a case by case basis and will not be appropriate in many circumstances (eg: short duration reactive maintenance repairs) and can also introduce additional hazards which must be considered. In a trussed roof space – The careful manner of working within a roof space must be adhered to. Only stand on the roof trusses and use the cross trusses to hold onto for additional stability. Have your foot correctly positioned on the adjacent truss before transferring your weight. Do not rush! Access into the roof space should be via footed extension ladder or an appropriate sized step ladder firmly placed on a level floor

A complete isolation to all electrical supplies running through the roof prior to entering the roof space is to be carried out. Remove lightweight insulated foil from roof space and discard in the skip bin provided or remove from site.

Before any circuits are energized, they must be visually inspected and tested as in accordance with AS/NZS 3017-2007. After the circuits have been tested and verified they can then be re-energized. Additional PPE considered: LV insulated rubber mat and insulated gloves.

After the sub circuits have been energized, any live tests specified in accordance with AS/NZS 3017-2007 can be carried out using correct testing equipment and procedure. Any isolation premits can be signed off. Additional PPE considered: LV insulated rubber mat and insulated gloves.

Emergency Procedures
Emergency	Procedure
Fire / Explosion	In the event of Fire or Explosion follow the emergency response plan as explained in the site-specific site induction. This will include: Assembly points Communications Responsible persons Emergency contacts Use of fire protection equipment
Electric Shock	Do not touch the victim or power source until confirmation the Electrical source as been removed. If safe to do so, isolate the Electrical supply immediately. If not possible to isolate the Electrical supply: If safe to do so, free the victim from the Electricity source using the insulated hook bar in the low voltage rescue kit. If not available use a dry insulated material such as a piece of cable or belt. When safe, approach the victim and determine whether they are breathing. Under take CPR if required and dial 000 for victim to seek medical attention. Such incidents should be reported to the regulator.
EWP Rescue Plan	In the event that a Person needing rescuing from a EWP at heights: Follow the manufactures instructions to release the hydraulic valves at ground level to slowly lower the EWP to the ground. Call 000 and request help from the Emergency services.
